Finalist Details
Hunter Primary Care has been serving the community for over 30 years, providing essential health services, including NDIS, mental health and Aboriginal and Torres Strait Islander care. Their NDIS Allied Health team, which has doubled since 2019, operates across NSW, including the Newcastle and Hunter regions. The team is known for offering trauma-informed, person-centred care with services tailored to individual needs.
One of their innovative services is the Shared Health Appointments (SHA) program, based on Lifestyle Medicine principles. SHAs allow participants to engage in group sessions, fostering peer support, social connection and mutual learning. This model has been especially beneficial for Aboriginal and Torres Strait Islander communities, supporting social and emotional wellbeing.
Feedback from SHA participants highlight the sense of belonging and encouragement the program provides. Many clients have expressed feeling less alone and more supported, with improved confidence in managing their health. Participants appreciate the safe space to share experiences and the program's positive impact on their mental health and lifestyle choices has been widely praised.
